Student Scheduling Questionnaire

Dear parent:

Your answers to the questions below will give me the information I need to schedule your son or daughter for a regular trumpet lesson time during the school year.  I realize that completing this form is time consuming, and I thank you for doing so.  When you have completed the entire questionnaire, please click the Submit button at the bottom of the page. Finally, please be patient.  I’m sure you understand that it takes some time (and aspirin) to put together a schedule like this.  I shall email you as soon as I have assigned your trumpeter a lesson time, and I expect to begin teaching the first week of October.

Please note that I teach 45-minute lessons, as experience has taught me that the 45-minute lesson is more productive.  Those who pay for a semester of 30-minute lessons will receive nine 45-minute lessons rather than fourteen 30-minute lessons.  Those who pay for a semester of 45-minute lessons will, of course, receive fourteen 45-minute lessons.  If you have any questions about this policy, please email me or give me a call at (978) 884-8358.

 

Possibilities

For each day and location listed below, please indicate every possible moment at which your trumpeter could be available for a lesson.  Please start with the earliest possible starting time and end with the latest possible ending time.  If there is a break in your trumpeter's availability, please separate the time slots with a comma (ex. 1:45-3:20, 5:10- 7:35).  I will ask you for your time and location preferences later, but let's start with what is possible.  I must stress the importance of being thorough in this step, as it is the only way to schedule 25 to 30 students efficiently.  Also, please don’t assume I know when school ends, how much travel time is needed, or how late in the day your trumpeter is able to function - just indicate the time frame during which your trumpeter could be in a lesson at Newton North or Newton South.  If a day or location is not possible, please leave that box blank.
